Age: 29 Breast Augmentation
This mother is 5’2″ and 110 pounds and interested in a breast enhancement. As seen on her pre operative pictures, she has lost superior fullness, her breasts are relatively small, and she has some chest wall and rib asymmetries. The goal was to increase breast fullness, particularly superiorly, adjust the size of her implants to accommodate her rib asymmetries and produce a natural, proportional result.
She was augmented with saline implants, 340cc on the left and 330cc on the right, placed beneath the muscle through a small incision in the breast crease.
Age: 34 Breast Augmentation
This is a 34 year old mother of two who is 5’7″ and 115 pounds interested in a breast enhancement. Her husband preferred larger implants, while she was partial to the smaller ones. She has small breasts with no ptosis (sag). She had relatively loose skin about her breasts. Her nipple-areola complexes are collapsed. Her chest wall below her breasts is quite flat and not sloped like many women.
She is seen here after a bilateral breast augmentation with 275cc LOW PROFILE silicone implants placed below her muscle through an incision in the crease at the bottom of her breasts. Several issues deserve comment; First, we chose a low profile implant because if she is electing for smaller implants, they have to be wide enough to span the width of her chest. If not, the implants could look somewhat “stuck on” or like the “hot and cold” knobs on the sink. High profile implants that are too small won’t adequately fill her out. It is common to choose a lower or moderate profile implant for the smaller implants and the higher profile for the larger ones (but not always). Second, take a look at her scars at the bottom of her breasts at six months after surgery. She is healing very well and soon they will be almost invisible. They are hidden in the natural breast crease. This is the most popular incision choice for many reasons (read my Breast Augmentation article on my website for details). Note that the lower profile implants don’t project as much, so there is not as much superior fullness. Also, the overall breast is a bit flatter in appearance than what we might see if she had opted for the more narrow high profile implants.
As I suspected in her case, she wishes she had gone larger. I told her that although I share her enthusiasm because of her impressive outcome, I feel the main reason why she enjoys such a beautiful outcome is because she chose the best size for her anatomy!
Age: 36 Breast Augmentation
This patient is 5’9″ tall and 150 pounds. She has pectus excavatum (her central chest is sunken or caved-in). Her breasts appear quite different. This is the most common congenital (born with) deformity of the anterior chest and is not uncommonly seen in my practice in milder forms.
We elected to perform an asymmetrical augmentation with silicone implants. Though an incision in the crease below the breast, I placed a 400cc implant on the left and a 600cc on the right. The result is imperfect yet very impressive. This case illustrates the power of breast implants alone to address a developmental deformity that is associated with very significant negative psychological ramifications. This procedure has essentially changed her life and she couldn’t be happier with her outcome.
Age: 47 Breast Augmentation
This is a woman who is 5’4″ and 117 pounds who desired a breast enhancement. Her breasts are non-ptotic (don’t sag), they are small and they are high on her chest wall. We elected to perform a bilateral breast augmentation with 325 cc silicone implants placed beneath the muscle through an incision in the breast crease.
The first thing you notice in her post op photos is the amazing difference. Her adolescent breasts now look full like a mature woman. The skin on her breast looks better and her nipple-areola complex have come alive.
Age: 49 Breast Augmentation
This is a mother of three, 5’4″ tall who was interested in a breast enhancement. She felt that now that her children are all on their own she wanted to do something for herself. She didn’t feel “whole” because of her small breasts. She has small, asymmetrical breasts. Her chest wall on the right side is lower than on the left.
We elected to perform a bilateral breast augmentation with silicone implants (400cc on the right and 350cc on the left), placed below the muscle through an incision in the crease beneath the breast. Her breasts look symmetrical and very appropriate in size for her body type. They appear very natural. She is very pleased with her decision to invest in a breast enhancement. Note on the lateral post operative view the mark on her skin from her perfect fitting bra. The underwire needs to sit in the crease below the breast and hug the lateral portion of the breast to “hold” the implant in its best position: See arrow below.
Age: 38 Breast Augmentation
This is a mother of five who desired a breast enhancement. Her breasts have fallen (ptosis), they have lost superior fullness and they are wide (which makes her look matronly and overweight). Although I suggested a breast lift and an implant she was quite nervous about not looking too big because of her family life.
We elected to perform a bilateral breast augmentation with 375cc high profile silicone implants placed below the muscle through an incision beneath the breast. She also had liposuction of her lateral breast and under her arms. She is seen here at four months after her procedure. Note the overall improvement, but with continued sagging of the lower portion of the breast. She says she sees now the benefit of a Mastopexy or “breast lift”. She says she looks “perfect” when she wears her bra (see photo). She wished she had listened to me when I recommended a larger implant that would have provided for more “lift” since she didn’t want a Mastopexy.
This case illustrates the issue of the benefits of an augmentation alone when a breast lift would have improved the results. She has no regrets and absolutely loves her new breasts and admits that she chose to go in stages because she didn’t know what to expect.
