Age: 50 Liposuction
This patient is 5 feet tall and weighs 160 pounds. She has two kids. She has struggled with her weight and says she simply doesn’t have the body that she used to have. We had very personal and intense discussions about her weight and the benefits and risks of surgery. I felt she was a fair candidate for a first stage liposuction procedure followed thereafter possibly by an abdominoplasty to remove excess skin and tighten the fascia. On examination I found her skin to be only of fair quality and that she had intraabdominal fat that made her abdomen protrude more than ususal. This fat could only be reduced with weight loss–liposuction and a tummy tuck will not completely flatten her abdomen because part of the problem is the fat on the inside of her abdomen (visceral fat).
She is seen here five months after a first stage liposuction where I removed 8500cc of fat (a large volume liposuction). She is now 148 pounds and is remarkably smaller and more shapely. Her abdomen is flatter not only because of her liposuction but also as a result of fat loss from inside her abdomen because of the additional weight loss. Her skin has some irregularities as you would commonly expect when you have 50 year old skin. She notes that it’s more difficult to be disciplined about weight loss the further out from surgery she is, but clearly understands the need for continued wieght loss. The struggle with weight doesn’t change after plastic surgery; it just seems easier when you enjoy the benefits of your investment. It’s easier to be motivated when you look and feel good.
Age: 29 Liposuction
We performed a liposuction of the trunk and extremities of this 5’5″ and 141 pound patient. I removed 6250 cc of fat. The results are quite dramatic and typical in a patient who has excellent skin. Note the pictures of her backside and her arms in particular (it almost doesn’t seem like it’s the same person!) She actually gained five pounds since her pre-operative weight and we insisted that she lose this in order to realize the best result. Appropriate weight control after liposuction is critical to a long term satisfactory result. Liposuction should not be used as an alternative to a sensible diet and exercise regimen. Large volume liposuction should only be performed by an expert with this procedure (removal of greater than 5000cc of total aspirate).
Age: 39 Liposuction
This mother of two, 5’6″ and 190 pounds, was initially interested in an abdominoplasty and breast reduction. I educated her at length about all the issues involved and I recommended that she consider a first stage liposuction, a second stage abdominoplasty, and a third stage breast reduction. Most importantly, we discussed weight loss and weight management. No surgery is offered if she does not plan on losing significant weight and living a healthier and happier lifestyle. She must invest in herself.
She is seen here four months after a first stage liposuction. She lost 24 pounds since her procedure and has donated bundles of her clothes to Goodwill because “they just fall off.” Note her dramatic shape improvement. Her abdominal skin has retracted so well that an abdominoplasty is not even being considered! We are planning on her bilateral breast reduction in the near future.
This is the same patient after a bilateral breast reduction that was performed after she healed from her liposuction (about three months).
Age: 24 Liposuction
This mother of two, 5’6″ and 190 pounds, has struggled with her weight since having her children. She was interested in body contouring plastic surgery options. She is seen her at about five months after a large liposuction. She has lost eleven inches on her waist, decreased four dress sizes, and has lost a little over twenty pounds since her procedure. She has changed her lifestyle considerably by eating better and working out on a daily basis.
There are several issues to consider in her case. Liposuction is not performed for weight loss. In her case, after very intense and frank discussions, I felt that her personality and commitment to success would work in her favor. She would have to work very hard to change her lifestyle so she will be able to continue to lose weight and maintain weight loss into the future. Weight gain would result in the recurrence of fat not only in the treated areas but elsewhere as well (fat travels phenomenon). As a general rule, it’s always better to lose the weight before entertaining plastic surgery. In her case, the procedure provided for the immediate result which then gave her the motivation and positive feedback necessary for a successful life changing outcome.
